Are you passionate about helping patients manage chronic conditions while working from the comfort of your home? We're looking for a detail-oriented and compassionate Licensed Practical Nurse (LPN) to provide support and coordinate care for chronic care management patients.

What You'll Do
Coordinate patient care - Manage a caseload of chronic care patients, ensuring clinical and non-clinical needs are met.
Identify and connect patients to community resources - Assist patients in accessing the care and services that best support their health.
Provide patient education - Help patients better understand and manage their chronic conditions.
Medication management - Monitor adherence, reconcile medications, identify potential concerns, and coordinate refills.
Arrange healthcare services - Ensure timely access to Home Health, DME, Home Infusion, and other critical resources.
Support patient communication - Answer patient questions and foster open dialogue to assess and address needs.
Manage referrals and scheduling - Assist in coordinating specialist appointments and follow-ups.
What You'll Need
✅ Active Multi-State/Compact LPN License (NLC) or LVN License
✅ Strong technical skills - Comfortable using Microsoft Office and electronic health records (EHR) systems
✅ Excellent communication - Strong written and verbal skills to engage with patients and healthcare providers

Bonus Points If You Have:
✔️ Experience using Epic EHR
✔️ Additional state-specific LPN licenses
✔️ Bilingual skills (Spanish preferred)

Compensation & Benefits
Pay: $21/hr - $28/hr (with opportunities for OT!)
